Sir,

I was admitted to the Government Civil Hospital on the 12th September 121 suffering from Boils on the neck, I remained there for 11 days.

I was again admitted on the 9th December 191 suffering from Diptheria, I remained there for 20 days.

I was again admitted to Goverment Civil Hospital on the 5th February 1922 from suffering from severe Cold and Tonsilitas, I remained there for 5 days.
